This meeting is a convening of professionals dedicated to efficient, effective care coordination across various healthcare settings. This gathering unites the spectrum of care levels and professional roles including liaisons, hospital case management + social work, alongside those managing care teams and facilities in post-discharge settings.
Vendor Tables (18) will be available on a first come basis benefitting the Mount Carmel Foundation via suggested donation of $250. The Mount Carmel Foundation was founded in 1984 as an Ohio 501 (c)(3) non-profit organization dedicated to funding the mission-driven health and education programs and services provided through the Mount Carmel Health System.
